Longevity habits, ranked by how much healthspan you get per hour
If you could only spend three hours a week on staying alive longer — genuinely three, not the fantasy schedule where you also meal-prep, ice-bath and meditate — where should those hours go? Most longevity content answers a different question. It ranks habits by effect size: this lowers mortality 40%, that lowers it 30%, here’s a list of nineteen things, good luck. Which is useless if you have a job, because effect size ignores the one resource you’re actually short of. Not motivation. Time.
So I did the slightly obsessive thing and re-ranked the usual longevity habits by a different axis: healthspan per hour. Not “how big is the benefit” but “how big is the benefit divided by the weekly hours it costs you.” It turns out this reshuffles the leaderboard quite dramatically. Some beloved biohacks fall off a cliff. And the thing that ranks near the very top costs, on a good week, no extra hours at all.
The thesis, up front: with only a few hours a week, strength training, Zone 2 cardio and sleep regularity beat everything else — and the fashionable stuff, sauna and cold and the supplement drawer, gives you far less per hour than its fans think.
How I ranked them
One axis: mortality and healthspan benefit relative to the discretionary weekly time the habit costs. Two things fall out of that immediately, and they force the list into two tiers.
First, diminishing returns are the whole story. Almost every one of these habits has a steep early slope — the jump from none to some is enormous — and then it flattens. So the per-hour return is highest at the first hour and drops after. A ranking by time has to reward the habits that deliver most of their benefit in the first hour.
Second, some habits cost essentially no discretionary time at all — you’re already spending the hours (sleep) or just redistributing them (not sitting). Divide a real benefit by a denominator of roughly zero and the ratio explodes; strictly by the formula these would top the list by a mile. That’s not a useful ranking, so I split them out: I rank the habits that cost real discretionary hours strictly by benefit-per-hour (that’s the #1–#2, #5–#9 spine), and I slot the near-free habits (sleep, not-sitting) by their absolute benefit and by one honest dependency — they only compound once you’re actually doing the training underneath. A perfectly regular sleep schedule on top of no exercise buys far less than the same sleep on top of two lifts and a couple of walks. So “#1 best return on time” means best among the habits that cost you hours; the near-free habits ride just behind the two training essentials rather than leapfrogging them on a technicality.
A caution before the list, same as always: these are population associations, and some of the strongest ones (fit people, good sleepers) partly reflect that already-healthy people can do these things — reverse causation. I’ll flag it where it bites. Right, the ranking.
1. Strength training — the best return on time, full stop
Two sessions a week. Call it 90 minutes total. For that, a 2022 meta-analysis in the British Journal of Sports Medicine (Momma et al.) found muscle-strengthening activity associated with a 10-17% lower risk of all-cause mortality — and here’s the part built for this ranking: the benefit peaked at around 30-60 minutes a week and then flattened. You read that right. The mortality return maxes out at under an hour. There is almost nothing else on this list that gives you that much for that little.
It’s not just the mortality number. Strength is the one thing that directly defends against sarcopenia and frailty — the slow muscle loss that decides whether your last decades are independent or not. That’s healthspan, not just lifespan. This is the whole case for picking up heavy things as you age, and per hour you actually spend, it is simply the best deal in longevity.
2. Zone 2 cardio — the aerobic base, where the first hours pay most
Aerobic fitness (VO2 max) is the single strongest predictor of mortality we have, and the return on the first chunk of it is absurd. Arem et al. (JAMA Internal Medicine, 2015), pooling 661,000 adults, found the steepest mortality drop came from just meeting the guideline 150 minutes a week — with most of the benefit already banked well before that, and only modest gains from doing 3-5x more. The dose-response is front-loaded: the person going from nothing to a couple of easy hours a week gains far more per hour than the person going from five hours to seven.
Zone 2 — conversational-pace walking, easy cycling — is how you buy that base cheaply, with almost no injury or recovery tax. It’s the slightly boring training that actually works, and it costs more weekly hours than strength, which is the only reason it sits at #2 rather than #1.
3. Sleep regularity — the highest ROI precisely because it’s nearly free
Here’s the surprising placement. Sleep tops the near-free tier — it costs no extra discretionary hours, you’re spending the time in bed whether it helps you or not, so the per-hour return on improving it is close to infinite: the denominator is roughly zero. On the raw formula it would sit at #1; it lands at #3 because, per the two-tier method above, a near-free habit is placed by absolute benefit and by the dependency that it only compounds on top of the training underneath it.
And the improvement worth making is cheaper than “sleep more.” A 2023 study of ~60,000 UK Biobank participants (Windred et al., Sleep) found that sleep regularity predicted all-cause mortality more strongly than sleep duration did — the most regular sleepers had roughly 20-48% lower mortality risk than the most irregular. Going to bed and waking at a consistent time is a scheduling change, not a time cost. That’s why when you sleep beats how long, and why it earns a podium spot on a list that’s otherwise about how you spend your gym hours.
4. Not sitting all day — the free habit hiding between the others
This one costs no dedicated hours either; it’s about redistributing time you’re already using. Prolonged sitting tracks with higher mortality, but Ekelund et al. (The Lancet, 2016), pooling over a million people, found that roughly 60-75 minutes of daily activity blunted most of the excess risk from long sitting — and even breaking up sitting with light movement helps. You don’t need a dedicated block; you need to stop being still for eight unbroken hours. Standing, walking to the U-Bahn, taking the stairs. Nearly free, genuinely protective — the flip side of everything in why sitting is quietly costing you.
5. Eating well enough — big effect, real hours
Diet has a large effect size — PREDIMED (Estruch et al., NEJM, 2018 reanalysis) showed a Mediterranean pattern cut major cardiovascular events by ~30% in a randomised trial. So why is it mid-table on a per-hour list? Because doing it well costs real, recurring time: shopping, cooking, planning. The good news is the highest-ROI dietary moves are cheap in effort — enough protein, more fibre, fewer ultra-processed foods — so you can capture most of the benefit without becoming a person who spiralises things. 6. Sauna — real data, softer than the number looks
The Finnish KIHD cohort (Kunutsor & Laukkanen, JAMA Internal Medicine, 2015) is genuinely impressive: 4-7 sauna sessions a week associated with ~40% lower all-cause mortality versus one. Big effect. And, to be fair, not a huge time cost — the cohort’s average session was about 15 minutes, so 4-7 a week is only ~1-2 hours. So why does it rank below the training habits rather than above them? Because the numerator is shakier than it looks. This is a single observational cohort, and much of the benefit plausibly rides alongside the fact that fit, healthier men sauna more (confounding and reverse causation) — you can’t randomise a country’s saunas. A ~40% association you can’t lean on is worth less per hour than a 10-17% effect that shows up in randomised strength trials. Lovely if you enjoy it; just not where your first three hours go.
7. HIIT — efficient per session, no clear survival edge over easy cardio
Intervals are time-efficient for raising VO2 max, which is why people reach for them. But for the outcome that matters here, the Generation 100 trial (Stensvold et al., BMJ, 2020) randomised ~1,500 older adults and found no statistically significant all-cause mortality advantage of high-intensity intervals over moderate training across five years. So HIIT’s per-hour fitness return is high, but its per-hour mortality return isn’t clearly better than the gentler Zone 2 base — at a higher injury and recovery cost. 8. Cold plunge — the thinnest evidence on the list
The plunge is everywhere right now, so it earns a placement rather than a hand-wave. The honest problem is the numerator again, only smaller: there’s decent evidence cold exposure does short-term things — a noradrenaline spike, a mood lift, possibly a small metabolic nudge — but there is no hard human mortality or lifespan data behind it, nothing remotely like the KIHD sauna cohort, let alone a randomised strength trial. So you’re dividing an unproven longevity benefit by a real cost in time, discomfort and the faff of cold water. That’s a low number almost by definition. It sits down here for the same reason sauna slipped: the effect you’d be buying isn’t established. Fine as a thing you enjoy or use to feel alert; the full state of play is in what cold water immersion actually does. Just don’t file it above lifting.
9. The supplement drawer — nearly zero time, nearly zero return
Supplements cost almost no time, so you’d think they’d rank well on a per-hour axis. The problem is the numerator. The VITAL trial (Manson et al., NEJM, 2019) randomised ~26,000 people to omega-3 and vitamin D and found no significant reduction in the primary cardiovascular or cancer endpoints. Creatine has decent evidence for muscle and maybe brain, but it’s an adjunct to training, not a longevity lever on its own. When you divide a very small effect by a very small time cost, you still get a small number. Fine to take; just don’t file them under “longevity habits” and feel you’ve done the work — see what supplements actually earn their place.
The pattern, and how to use it
Stand back and the leaderboard tells one clean story: the habits with the best return on time are the physically demanding, slightly unglamorous ones — lift, walk, sleep on a schedule — and the fashionable add-ons buy less per hour than their marketing implies. The sauna and the plunge and the pill aren’t wrong; they’re just further down a list you should fill top-first.
So if you have three hours a week, here’s the defensible split, straight off the top of the ranking:
- ~90 minutes: two strength sessions. The single best return on this list, and it caps out under an hour of effective work anyway.
- ~60-90 minutes: Zone 2. Walk fast, cycle easy. Front-loaded benefit — the first hours are the valuable ones.
- Zero extra hours: sleep regularity. Same bedtime, same wake time. It’s a scheduling decision, not a time cost.
- Scattered, also free: don’t sit for eight unbroken hours. Movement snacks between the above.
That’s the whole engine, and it fits the four pillars of longevity training almost exactly — because when you rank by time instead of by hype, the pillars are what’s left standing. Everything below the top four is a genuine bonus once those are locked, not a substitute for them. The uncomfortable, freeing takeaway: the best longevity routine is shorter and duller than the internet wants it to be. Right, so — lift twice, walk often, sleep on a schedule. The rest is seasoning.
