Say you had a strict budget for keeping your muscle as the decades pile up — one intervention from the supplement shelf, one thing to actually think about. Creatine or protein? It’s the question I get asked more than almost any other, usually phrased as “do I really need both, or is one of them a bit redundant?”

It’s a reasonable question and it has an annoying answer, which is that they’re not actually competing for the same job. Asking whether creatine or protein is better for muscle is a bit like asking whether bricks or bricklayers are better for building a wall. You can have a lorry-load of one and get nowhere without the other. But “you need both” is a cop-out if I stop there — because if someone genuinely forced the ranking, there is a right order. So let me actually make the case, mechanism by mechanism, and then tell you which one I’d protect first.

The short version, so you can leave now if you like: protein is the floor you cannot skip, and creatine is the cheap multiplier that only pays off once the floor is in place.

Why this even matters

Both of these are muscle interventions, and muscle is the thing worth protecting. Holding onto muscle mass as you age is one of the more consistently documented predictors of a long healthspan — Srikanthan and Karlamangla’s analysis of the NHANES III cohort (The American Journal of Medicine, 2014) found higher muscle mass index was inversely associated with all-cause mortality even after adjusting for cardiovascular risk, metabolic markers and body fat. The enemy is sarcopenia: the slow, quiet loss of muscle and strength that starts in your forties and accelerates, and that eventually decides whether you can get off the floor unassisted at 80.

So the real question isn’t “which supplement is trendier.” It’s: of the two nutritional levers people reach for to fight sarcopenia, what does each actually do, and where should your limited attention go?

What each one actually does

Here’s the bit that dissolves the whole “which is better” framing, because they operate through genuinely different doors.

Protein is the building material. Dietary protein supplies amino acids, and one of them — leucine — acts as the switch. When blood leucine rises past a threshold (roughly 2–3g in a meal), it flips on mTORC1, the signalling pathway that triggers muscle protein synthesis. No adequate protein, no raw material and no trigger. Simple as that. And ageing muscle is harder to switch on: Cuthbertson and colleagues (FASEB Journal, 2005) gave graded doses of amino acids to young and older men and found the older muscle mounted a blunted synthesis response to the same dose — a phenomenon called anabolic resistance. The practical upshot is that older adults need more protein, not less, and they need it in bigger per-meal hits (around 30–40g) to clear the higher threshold. The PROT-AGE consensus (Journal of the American Medical Directors Association, 2013) landed on 1.0–1.2g/kg/day for healthy over-65s and 1.2–1.5g/kg for active ones — well above the old 0.8g/kg RDA, which was only ever a deficiency floor. Creatine is the energy to use the material. Creatine has nothing to do with supplying amino acids. About 95% of your body’s creatine sits in skeletal muscle as phosphocreatine, a high-energy compound that rapidly regenerates ATP — the actual currency your muscles spend during hard efforts. More phosphocreatine means more capacity to push a heavy set, recover between sets, and generally train with enough intensity to force an adaptation. And here’s the ageing angle: muscle creatine stores fall with age (slower synthesis, less meat in the diet, less muscle to store it in), so there’s more empty tank to top up. The muscle benefit is real but conditional — Devries and Phillips’ meta-analysis (Medicine & Science in Sports & Exercise, 2014) and Chilibeck and colleagues’ meta-analysis of older adults (Open Access Journal of Sports Medicine, 2017) both found creatine plus resistance training beat resistance training plus placebo for lean mass and strength. Notice what just happened. Protein is substrate and signal. Creatine is energy. They don’t overlap. Which means their effects stack rather than cancel — and it means “which is better” was always the wrong question.

The head-to-head, on the axis that actually matters

If they do different jobs, how do you rank them at all? By asking the only question that decides the ranking: which one, if you got it wrong, quietly wrecks everything else?

That’s protein, and it’s not close. Protein is a hard prerequisite. If you’re eating 60g a day at 75kg — which is roughly where a lot of people actually land when they finally track it honestly — your muscle synthesis machinery is running short on raw material every single day, and there is no supplement that patches that hole. Creatine won’t. Creatine has no meaningful protein-sparing effect; it supplies zero amino acids. You can be perfectly saturated with creatine and still lose muscle if the protein isn’t there, because you’ve fuelled the bricklayers and forgotten to deliver the bricks.

Creatine, by contrast, is a conditional benefit. Look again at what the trials are actually testing: creatine versus placebo, on top of resistance training, in people already eating a normal diet. Its measured effect is the extra you get once training and adequate nutrition are handled. Strip out the training and the effect shrinks; the meta-analyses are clear that the impressive numbers come from the combination. Creatine is a multiplier, and a multiplier does nothing to a number that’s near zero.

So the honest hierarchy is:

  1. Enough protein — the non-negotiable substrate. Get this wrong and nothing downstream works.
  2. Resistance training — the stimulus that tells the body to keep the muscle (creatine’s benefit is largely parasitic on this; see the resistance training case).
  3. Creatine — the cheap amplifier that makes 1 and 2 pay off a bit more.

Now — the honest caveat, because this is the part that keeps the comparison from being propaganda. There is remarkably little research directly racing creatine against protein. The two live in separate literatures: creatine trials usually fix everyone’s diet and test the powder against placebo; protein trials rarely bother adding creatine as a variable. So this “vs” is built on mechanism and on trials that combine the two, not on a clean head-to-head where one arm gets creatine and the other gets extra protein. The mechanistic case that they’re complementary is genuinely strong. A definitive “creatine beats protein” or “protein beats creatine” trial basically doesn’t exist, and anyone claiming otherwise is overselling. Fair enough to say so.

But creatine is easier — isn’t that the point?

This is the strongest argument for leading with creatine, and it deserves a fair hearing. Creatine is genuinely the more convenient intervention. It’s 5g of a cheap white powder, once a day, mixed into whatever you’re already drinking, no thinking required. Hitting a protein target, by contrast, is a daily logistics problem — 30–40g at breakfast when most people eat toast, remembering it at lunch, not backloading the whole day into one enormous dinner. On pure effort-per-gram, creatine wins comfortably.

But “easier” and “more important” are different axes, and it’s worth not confusing them. The ease of creatine is exactly why it’s such a good addition — there’s almost no reason not to take it. It’s a terrible reason to take it instead of fixing protein, because you’d be optimising the multiplier while ignoring the base number. The move isn’t “creatine because protein is a faff.” It’s “creatine because it’s a free win, on top of the protein you’re going to sort out anyway.” The convenience is a point in creatine’s favour as an add-on, not an argument for reordering the hierarchy.

Who should prioritise which

Since the real-world answer is “both, in the right order,” picking mostly means knowing which gap is yours.

  • If you have no idea what your daily protein actually is: start there, and don’t touch creatine yet. Track honestly for a week. Most people over 40 discover they’re at 0.8–1.0g/kg when they think they’re “eating loads of protein.” Closing that gap is the single highest-yield change on this page, and it costs nothing but attention.
  • If your protein and training are genuinely dialled in: add creatine. This is precisely the person the trials are describing — adequate diet, resistance work, and creatine on top for the extra lean-mass and strength edge. 5g a day, done.
  • If you’re plant-based: you’re the one person for whom creatine punches above its usual weight. Dietary creatine comes almost entirely from meat, so vegetarians and vegans run lower baseline muscle stores and tend to respond more strongly to supplementation.   • If you’re over 70: both, and with more urgency than at 40. Anabolic resistance means the protein has to be higher and better-distributed, and lower baseline creatine stores mean the top-up is more useful. This is the age where the chair-stand-test improvements in the creatine-plus-training trials stop being abstract.

The verdict

Creatine versus protein is a false fight, the same way Zone 2 versus HIIT is — the honest answer is “both,” but the useful answer is the ranking underneath it. They target different bottlenecks: protein is the building material and the signal, creatine is the energy to build. Because they don’t overlap, they stack, which is why nearly every serious muscle-preservation plan quietly includes both alongside strength training.

But if something forced the choice, protein wins on importance every time, because it’s the prerequisite the whole system fails without — and creatine wins on convenience, which is exactly why it makes such a good addition rather than a substitute. Get protein to 1.2–1.6g/kg spread across your meals, do your two or three strength sessions, and then spend the couple of euros a month on 5g of creatine monohydrate to squeeze a little more out of both. The order matters more than the ingredients: fix the floor, then add the multiplier.

The most common mistake I see isn’t people neglecting creatine. It’s people buying the tub of creatine, feeling like they’ve done the muscle thing, and never once checking that they’re eating enough protein to make it matter. Creatine on top of an inadequate diet is a multiplier on a small number. Sort the number first.
