Does collagen do anything for aging? An honest look at the evidence

Most people think collagen is an anti-aging supplement. Turns out it’s a skin-and-joints supplement wearing an anti-aging costume — and the costume is doing a lot of heavy lifting.

Collagen is everywhere now. It’s in the coffee creamer, the protein powder, the £40 tub with a woman doing yoga on the label, and roughly forty percent of the wellness reels I get served at 11pm. The pitch is always the same and always vague: collagen for aging. Which sounds like it means one thing but is quietly doing the work of about five — younger skin, better joints, stronger bones, and, if you don’t look too closely, something like a longer life.

So let me do the annoying thing and look closely. Because there genuinely is real evidence here. It’s just evidence for a much smaller, more boring claim than the one on the tub.

The claim, as everyone states it

The everyday version goes: your body’s collagen declines with age — true, it does, by roughly one percent a year after your mid-twenties — and that decline shows up as wrinkles, creaky joints and thinning bones. Therefore, if you top up the collagen by drinking it, you slow the whole process down. Anti-aging in a scoop.

It’s a tidy story. The problem is the middle step, the “therefore,” which quietly assumes that collagen you swallow ends up as collagen where you want it. That’s not how protein works, and it’s where most of the magic hides.

Where it came from

The modern collagen boom is genuinely built on a real research base — this isn’t homeopathy. Over the last fifteen years a run of randomised controlled trials tested hydrolysed collagen peptides (collagen broken into small fragments so it actually absorbs) and found measurable effects. That’s the important bit to be fair about: there are real RCTs, with placebo groups and objective measurements, and several of them are positive.

The catch is what they measured, how long for, and — we’ll get to this — who paid.

What the strongest studies actually show

Let me steelman it properly, because the good studies deserve it.

Skin. The most-cited trial is Proksch and colleagues in Skin Pharmacology and Physiology (2013): 69 women aged 35-55 took 2.5 g or 5 g of collagen peptides or placebo daily for eight weeks. Skin elasticity, measured objectively with a probe, improved significantly versus placebo in both dose groups. A later study by Kim and colleagues in Nutrients (2018) gave 64 people 1 g of low-molecular-weight collagen peptide daily for twelve weeks and found significant improvements in skin hydration, elasticity and wrinkle depth. There are now enough of these that a modest, real skin effect is about the most defensible claim collagen has. (According to PubMed: Proksch et al., DOI; Kim et al., DOI.)

Joints. Clark and colleagues ran a 24-week trial at Penn State, published in Current Medical Research and Opinion (2008): 147 athletes with activity-related joint pain took 10 g of collagen hydrolysate or a placebo. Several pain measures — joint pain when walking, standing, lifting — improved significantly on collagen, and more so in the subgroup with knee pain. Not a cure, but a real, if modest, reduction in soreness in active people. (According to PubMed: Clark et al., DOI.)

Bone. König and colleagues in Nutrients (2018) gave 131 postmenopausal women 5 g of specific collagen peptides or placebo for a full year and measured bone mineral density by DXA. The collagen group’s spine and femoral-neck density rose significantly relative to placebo, alongside a favourable shift in bone-turnover markers. A year-long trial with a hard-ish endpoint is genuinely the strongest study in the pile. (According to PubMed: König et al., DOI.)

Muscle. Zdzieblik and colleagues in the International Journal of Environmental Research and Public Health (2021) had 97 middle-aged men do 12 weeks of resistance training while taking 15 g of collagen peptides, whey, or placebo. The collagen group gained more fat-free mass and lost more fat than placebo. Interesting — but note the framing: this only worked bolted onto lifting, and even here whey wasn’t clearly beaten. That’s the honest best case. It’s not nothing. Skin elasticity, joint comfort, and — with the strongest single study — bone density. If someone tells you “collagen is completely useless,” they haven’t read the trials either.

The honest verdict: partial for skin and joints, hype for “aging”

Here’s where I stop being generous, because three things about that evidence pile matter enormously and never make it onto the tub.

One: nobody has ever measured longevity. Not a single one of these trials looked at how long anyone lived, or any hard health outcome that predicts it. They measured skin-probe readings, pain questionnaires, and bone density. Those are cosmetic and comfort outcomes — worth having, but they are not “anti-aging” in the sense the marketing wants you to hear. When a supplement is sold on the word aging but every study stops at wrinkles, that gap is the whole story. If you want to know what actually tracks with aging at the whole-body level, biological age is measured by things like inflammation, glucose and fitness — none of which anyone has shown collagen touches.

Two: the studies are small, short, and heavily industry-funded. Look at the author lists and funders across those papers and the same handful of collagen research institutes and ingredient manufacturers keep appearing — the people who make the collagen are, over and over, the people testing it. That doesn’t make the results fabricated. But industry-funded nutrition research reliably skews toward favourable results, publication of the null trials is patchy, and “n=64 for twelve weeks, funded by a peptide company” is a very different evidentiary weight than the multi-year, independent cohort data behind things like fitness or protein. Real signal; thin, conflicted evidence base. Both things are true at once.

Three: the mechanism isn’t “eat collagen, get collagen.” This is the myth’s load-bearing wall and it’s wobbly. When you drink collagen, your gut dismantles it into amino acids and small peptides exactly like any other protein — it does not sail through your bloodstream intact and reassemble itself in your face. The more honest mechanism, and the one researchers actually propose, is that a few specific collagen-derived dipeptides (proline-hydroxyproline and friends) survive digestion, circulate in small amounts, and may act as signals nudging your own cells to make more collagen. That’s a real hypothesis with some support. It is also a lot more modest, and a lot less certain, than the picture in your head of collagen “topping up.”

There’s a fourth, quieter point too: collagen is a bad protein in the ordinary sense. It’s missing tryptophan entirely and is low in several essential amino acids, so as a muscle-feeding protein it’s near the bottom of the list. If you’re spending your protein budget on collagen thinking it doubles as muscle insurance, it mostly doesn’t — that’s what dietary protein is for, and collagen is a poor substitute for it.

What to do instead

I’m not going to tell you to bin your collagen. If you’re taking it, it’s cheap-ish, it’s safe, and for skin and joints you might get a modest real benefit — that’s a perfectly reasonable thing to spend money on with your eyes open. Give it a proper three months before you judge, because nothing in those trials separated from placebo quickly.

But if what you actually want is the aging part — the longer, stronger, more capable-in-your-seventies part — collagen is nowhere near the top of the list, and pretending it is just means you’re optimising the wrong scoop. The things with real longevity evidence behind them are the unglamorous ones:

  • Enough total protein from complete sources. This does more for aging muscle than collagen ever will, and it’s the actual fix for the thing collagen is quietly failing to do. Start with protein intake and longevity.
  • Resistance training. For bone density — collagen’s strongest claim — lifting has a vastly larger, more independent evidence base than any peptide. Bones respond to load.
  • Creatine, if you want a supplement with better odds. It’s one of the few with genuinely robust, largely independent evidence for muscle and possibly brain as you age — a much stronger bet per pound than collagen. Here’s the honest read on creatine.

So, the verdict, stated plainly: collagen has a modest, real effect on skin elasticity and joint comfort, a decent single study on bone, and — this is the whole point — zero evidence for longevity, built on a small and manufacturer-heavy research base. It’s a fine skincare-and-joints supplement being sold as an anti-aging one. Take it for what it actually does, not for the word on the label.

Honestly, that’s most of wellness in one tub: a small true thing, wearing a much bigger claim, priced accordingly.
